The CTLS requires that individuals who require a security clearance submit their security clearance application form before a licence application can be submitted in the CTLS. As such, we suggest to have applicants obtain their criminal record checks and apply for their security clearance, if applicable, as soon as is feasible. A licence will not be granted unless required security clearances have been granted.

As part of the Application to become a Licence Holder under the Cannabis Act and its regulations and in the case of changes of personnel, a Security Clearance Application, including the security clearance applicant’s fingerprints must be submitted for all individuals that require a security clearance. All security clearance applicants must complete a Security Clearance Fingerprint Third Party Consent to Release Personal Information form.

create a Cannabis Tracking and Licensing System (CTLS) account


Health Canada has established the CTLS as the primary manner in which licensing applications should be submitted. The first step to create an application is to set up an individual user account in the CTLS. To request an account, basic information is required including full name and salutation, email, phone number, date of birth, language preference and security information. Health Canada then provides an access code that can be used to enter the CTLS. Once an account is established, the user will have an Account ID in the CTLS.

NOW THAT YOU HAVE CREATED AN ACCOUNT AND ARE LOGGED INTO YOUR CTLS ACCOUNT, SCROLL DOWN TO SECURITY CLEARANCE APPLICATION. YOU WILL BE INSTRUCTED TO PROVIDE THE FOLLOWING INFORMATION TO COMMENCE YOUR SCREENING:

  • BIOGRAPHICAL INFORMATION

  • CRIMINAL CONVICTIONS

  • RESIDENTIAL ADDRESSES (LAST 5 YEARS)

  • EMPLOYMENT, EDUCATION AND UNEMPLOYMENT HISTORY (LAST 5 YEARS)

  • MARITAL STATUS (LAST 5 YEARS)

  • TIME SPENT OUTSIDE OF COUNTRY OF RESIDENCE (LAST 5 YEARS)

  • SIGNED CONSENT (STEP 3 FORM)

Security Clearance Fingerprint Third Party Consent to Release Personal form


now that you have created an account in step 1, As part of the criminal record check process, the applicant must provide a “Security Clearance Fingerprint Third Party Consent to Release Personal Information Form” which is provided below. This must be brought to the local police force, the RCMP, or a private fingerprinting agency accredited by the RCMP. The form authorizes the RCMP to release the criminal record check and fingerprint verification results to Health Canada. Following fingerprinting, a document control number (DCN) is provided on the form, which is used as the identifier for the record check, in accordance with these following instructions:

1. The form below is to be brought with the applicant to the local Police Station or RCMP private accredited fingerprint agency where their fingerprints will be taken.

2. This form must be completed in front of the person taking the fingerprints.

3. It is very important that a single digit ink print be placed in the box on the form below and the finger used identified by circling the appropriate one next to the box. Failure to do so will result in delays.

4. The form below must be signed and dated by the applicant.

5. The form below must be submitted to the RCMP at the same time as fingerprints taken on *Form C-216C at your local Police Station, RCMP or private accredited fingerprinting agency, accredited by the RCMP, whether electronically or by mail (in case of roll and ink prints).

*Form C-216C is the standard RCMP form used for fingerprinting

• This form is not available online

• You must have your fingerprints taken on Form C-216C at your local police station or private fingerprinting agency accredited by the RCMP. Please see this link for a list of accredited private fingerprinting agencies

• Depending on the police jurisdiction, a fee may be required

NOTE: To avoid any delays, an ink print MUST be placed in the box below in order for your prints to be processed in a timely manner. Please circle the finger used for the Biometric Consent as well.

now that you have completed your “Security Clearance Fingerprint Third Party Consent to Release Personal form”, it is on to your final step. the form below provides consent to the disclosure by law enforcement agencies to Health Canada and/or the RCMP of any and all information relevant to this security clearance application, including information in your criminal record and any other information contained in law enforcement records, including information gathered for law enforcement purposes, as well as any and all information that will facilitate the conduct of a security assessment.
